ATN Community Health Foundation is dedicated to providing comprehensive support to individuals and families experiencing homelessness in Orange, Osceola, Seminole, and Miami-Dade counties, Florida. Our goal is to ensure that everyone has access to safe and stable housing and the necessary resources to maintain a healthy and independent lifestyle.

1. Rapid Rehousing
   
   • Objective: Quickly move individuals and families from homelessness into permanent housing.
   • Services: Short-term rental assistance, housing search and placement, and case management.

1. Permanent Supportive Housing
   
   • Objective: Provide long-term housing solutions with supportive services.
   • Services: Long-term rental assistance, on-site support services, and case management for individuals with disabilities or chronic homelessness.

1. Transitional Housing
   
   • Objective: Offer temporary housing solutions while individuals work towards stability.
   • Services: Temporary housing, support services, life skills training, and employment assistance.

1. Emergency and Long-term Supportive Services
   
   • Objective: Address immediate and long-term needs of those experiencing homelessness.
   • Services: Emergency shelter, food assistance, healthcare services, mental health counseling, and substance abuse treatment.

1. Ongoing Case Management
   
   • Objective: Ensure sustained stability and independence for program participants.
   • Services: Continuous support, goal setting, resource linkage, and advocacy.

ATN Community Health Foundation recognizes that combating homelessness requires a collaborative effort. We actively partner with other organizations and agencies to enhance the effectiveness of our programs and extend our reach.

• Local Government Agencies: Collaborating with county and city housing authorities to access additional resources and support services.
• Non-Profit Organizations: Partnering with local non-profits like the Coalition for the Homeless of Central Florida, Second Harvest Food Bank, and Heart of Florida United Way to provide comprehensive services.
• Healthcare Providers: Working with community health centers and hospitals to address the health needs of the homeless population.
• Educational Institutions: Partnering with local schools and colleges to provide educational opportunities and vocational training.
• Corporate Sponsors: Engaging with businesses for funding, volunteer support, and in-kind donations.
